Welcome to the Middle School Church Camp webpage for the Ohio Cohorts camp. We are glad that your son or daughter will be joining us this year.
Below you will find various forms and information you may need before, or during, your child's week of camp.

BEFORE YOU BEGIN: Make sure you have your medical/insurance information handy. You will be asked for this information, in lieu of filling out paper medical forms.
CHECKING OUT: Once you have completed the forms, remember to check out at the very end to complete your registration.
REGISTRATION CONFIRMATON: You will receive a confirmation email once your registration has been successfully submitted.
